Stronger….Together: How and Why to Join a Behavioral Health Clinically Integrated Network (CIN)
Join November 15th at 12pm – 1pm to hear about the advancement of behavioral health clinically integrated networks that partner with primary care, specialists, and hospitals to improve coordinated access to care and improve communication. These ONEcare Networks by Alera Health are provider-governed, professionally managed, and partner with payers, managed care organizations, hospitals, and other Accountable Care Organizations in pay-for-performance programs which incentivize improved patient health and mental health outcomes; lowering ED/Inpatient utilization by as much as 70% while cutting 30-day readmission rates by 50%. To automate and enable more than 13 networks nationwide, Alera has assembled and deployed a suite of analytic and network technology solutions designed to detect undiagnosed behavioral health disease, identify and communicate Care Alerts relating to patient risk, and measure network and provider performance. Leveraging automated referral coordination and mobile patient monitoring tools, patients can quickly be connected with an integrated care team which includes behavioral health, medical, and social care organizations. We know that 70% of health outcomes are driven by social determinants of health and individual behaviors, says Mike Rhoades, CEO of Alera Health, The adoption of clinically integrated networks leading with behavioral health and social determinants of health expertise is a no brainer in terms of sustainable health and cost outcomes.
Built on system of care principles and utilizing a shared technology infrastructure, behavioral health provider agencies are finding themselves at a competitive advantage to engage in value based reimbursements, including shared risk arrangements by participating in a clinically integrated network. Health Plans are more likely to align themselves with comprehensive provider systems–ones that have comprehensive statewide, or even multistate coverage, where expertise from many provider agencies can be leveraged. “There is power in numbers–collective and shared expertise and resources”, says Carol Clayton, PhD, Senior Consultant with OPEN MINDS, “what providers can do together, they can not necessarily do alone”.
For those in recovery, experiencing mental health disorders or negatively impacted by social or economic distress, the clear evidence is that life and health outcomes are interdependent on a variety of community resources. This concept, known as system of care is foundational for specialty providers. Leveraging shared expertise, training, operational procedures and technology infrastructure creates a system of care model that is repeatable, reliable and sustainable and supports value based contracting. Fee for service delivery systems are not sustainable over time for complex needs consumers with significant behavioral, economic and social challenges.
