Breaking Down Silos: How One State Achieved 98% Faster Referral Response With Integrated Data & Coordination Tools

When referrals rely on phone calls, faxes and disconnected systems, individuals seeking behavioral health treatment can fall through the cracks. Delaware’s Division of Substance Abuse and Mental Health (DSAMH) faced this challenge head-on—transforming their fragmented systems use into a more connected, coordinated approach with interoperable data and real-time visibility.

Hear DSAMH’s success story on Tuesday, May 12 at 2:00 pm ET — during an executive-level webinar discussion on how cross-agency data sharing and innovative technology implementations can reduce duplication, close service gaps, and improve outcomes for high-need, high-cost populations.

Key Learning Objectives:

  • Discover tangible, best practice approaches to governance decision-making
  • Understand whole-person care privacy considerations
  • Learn the implementation steps for care coordination efforts
  • Gain clarity on how to build an executive roadmap for interoperable systems
