Vermont State Department of Health seeks a contractor to provide education, training, and, coaching to provider practices and local coaches in Blueprint communities using the Clinical Microsystems approach to quality improvement. This training will include two readiness Webinars which will be one hour in length and delivered prior to learning sessions. Purpose is to assure that teams and participants know expectations and activities for learning sessions and come prepared.
