West Virginia Department of Health, Bureau for Public Health, Office of Community Health Systems and Health Promotion, Division of Primary Care seeks a contractor to provide technical and organizational assistance to community-based care services throughout the state. The program shall create and administer a “Primary Care Grant” which will distribute funding to primary care centers in the form of grants designed to offset the centers’ cost of providing uncompensated health care services.
