The purpose of section D of the Ohio Medicaid Managed Care Request for Applications is to seek out applicants that have experience in delivering intensive care management interventions and enhanced care coordination to Medicaid beneficiaries. Care coordination strategies will ensure that patients and providers have access to accurate and timely information in order to optimize care.
