The Substance Abuse and Mental Health Services Administration in collaboration with the John D. and Catherine T. MacArthur Foundation released this application form for Improving Diversion Policies and Programs for Justice Involved Youth with Co-occurring Mental Health and Substance Abuse Disorders Initiative.
