CalAIM PATH Facilitator Awardees

The California Department of Health Care Services (DHCS) released the document on December 16, 2022. It is a list of the organizations selected to be facilitators for 25 county and regional collaboratives planning how to implement new Providing Access and Transforming Health (PATH) initiatives under the California Advancing and Innovating Medi-Cal (CalAIM) Medicaid waiver.
