As of February 2023, there are 13 states with Medicaid accountable care organization (ACO) models: Colorado, Delaware, Idaho, Iowa, Maine, Massachusetts, Minnesota, New Jersey, New York, Oregon, Rhode Island, Utah, and Vermont.

Since 2016, three states have added Medicaid ACOs—Delaware, Idaho, and Ohio. These ACOs fall into two main models, shared savings ACOs and risk-based ACOs.
