NQF’s Leadership Consortium Piloting Recommendations For Social Determinants Of Health Data In Real Health Care Settings
The National Quality Forum's (NQF) Leadership Consortium is piloting recommendations for using social determinants of health (SDOH) data in real world health care settings. SDOH are nonmedical factors that influence health outcomes. The Leadership Consortium members have prioritized making effective use of SDOH data to improve consumer outcomes and increase health equity, specifically by connecting consumers with community resources.
