Hawai’i Pacific Health Partners With Trellis Rx To Launch New Integrated Specialty Pharmacy & Disease Management Services

Hawai'i Pacific Health (HPH) launched a new disease management program and specialty pharmacy in an alliance with Trellis Rx, LLC. The new program is for HPH members living with chronic and complex health conditions. Trellis Rx is hiring local specialty pharmacists and pharmacy liaisons to support the new services.

Locally based specialty pharmacists and pharmacy liaisons will be embedded into HPH care teams.
