Texas-Based Catalyst Health Network Partners With agilon health

Catalyst Health Network, a large clinically integrated network for independent primary care professionals (PCPs) in Texas, entered a long-term partnership with agilon health inc. The partnership will bring agilon’s value-based, Total Care Model to Catalyst’s primary care network and build upon Catalyst’s model of relationship-driven care to improve outcomes and experience for older adults throughout Texas.
