On May 8, 2012, the Washington State Department of Social and Health Services published a proposal proposal to qualify community-based entities to deliver health home services at an effective level to Medicaid and dual Medicare-Medicaid eligible chronically ill, high risk individuals (the top 5% of the Medicaid population) and a documented future risk score (of higher health care costs) of 1.5 or greater.
