ACMH: An OPEN MINDS Organizational Profile

Founded in 1973, ACMH is a 501(c)(3) non-profit organization that provides transitional housing programs with a rehabilitative focus and permanent housing that provides both a financial subsidy and direct service supports for individuals with chronic mental health and substance use disorders and care management for adults with mental illness and other chronic medical conditions. ACMH operates programs, including Assertive Community Treatment and Crisis Residential Support, licensed by the New York State Office of Mental Health. Their mission statement is, “ACMH is committed to the mental and physical wellbeing of vulnerable New Yorkers.”
