Hawai’i Medicaid State Plan, 1115 Demonstration & 1915(c) Demonstration

On October 13, 2023, the state of Hawai'i published this Medicaid state plan for the 1115 Demonstration and the 1915(c) Demonstration. Pursuant to Section 1115(a) of the Social Security Act, the State of Hawai‘i Department of Human Services (the State) is seeking a five-year extension of the QUEST Integration Medicaid Section 1115 Demonstration from the Centers for Medicare and Medicaid Services (CMS).
