Food For Health Partners With ICare To Address Hypertension, Diabetes  

Food For Health, a Wisconsin non-profit organization, is partnering with Independent Care Health Plan (iCare), a Humana subsidiary, to offer medically tailored meals (MTM) and other services for certain Medicaid members who receive care through Milwaukee Health Services Clinics. The effort is focused on consumers with hypertension and Type 2 diabetes.;

ICare is a managed care organization that offers Medicaid and Medicare insurance benefits to people with disabilities and low incomes. It will work with Milwaukee Health Services to identify members who meet the enrollment criteria. ;
