Kaiser Permanente & Town Hall Ventures Partner To Launch Habitat Health  

Kaiser Permanente collaborated with Town Hall Ventures to launch Habitat Health, an organization designed to help older adults. Habitat Health is a Program of All-Inclusive Care for the Elderly (PACE), and it aims to support participants to live independently in their homes and communities. ;

Habitat Health plans to launch PACE sites in Sacramento and Los Angeles, California in 2025. It will serve older adults in partnership with local care partners. It aims to launch nationally later.
