Molina Healthcare Partners With Cityblock To Offer Dual Eligibles Increased SDOH Assistance 

Cityblock Health is partnering with Senior Whole Health by Molina Healthcare in Massachusetts. The collaboration will help Senior Whole Health's dual eligible members connect with community resources as needed to provide services that address needs for social determinants of health (SDOH).  

The program is targeting dually eligible members. According to the companies, more than 9,000 Senior Whole Health members will be eligible to receive resources possible from the partnership.
