On August 23, 2012, the Commonwealth of Massachusetts released an addendum to its proposal to the Center for Medicare and Medicaid Innovation to integrate care for dual eligible individuals. The addendum includes information on Home and Community-based Services (HCBS) waivers, Targeted Case Management, and the Rehab Option.
