Wisconsin 2012 Title IV-E Waiver Demonstration Application

The Wisconsin Department of Children and Families applied for a a federal foster care Title IV-E demonstration waiver to to launch a statewide demonstration project that will provide post-reunification case management and aftercare services for a year. The goal of the five-year demonstration is to prevent more than 1,000 children from reentering out-of-home care within the first 12 months after reuniting with their families. The federal Administration on Children and Families granted the waiver.
