Pennsylvania System of Care Partnership Expansion County Application

The System of Care Partnership is funded through a $9 million, six-year federal grant that is being rolled out to 15 counties, with the goal of eventually serving 1,000 Pennsylvania youth. Pennsylvania was one of 19 states chosen to receive the federal System of Care Expansion Planning Grant in 2011. In October 2012, the Pennsylvania System of Care Partnership released a request for county applications to become partners or learning communities. County responses were due by October 29, 2012.
