OneCare Vermont ACO To End Operations In December 2025

OneCare Vermont, a Medicaid accountable care organization (ACO) will end operations by December 31, 2025. This decision comes as the Vermont All-Payer ACO Model (VTAPM), a state-led initiative designed specifically for Vermont, is also set to end at the end of the calendar year 2025. In the closure announcement, OneCare Vermont said all programs, investments, and waivers will operate as previously planned to facilitate a smooth transition as participants explore future opportunities for 2026.
