Pyx Health Launches Health Condition Management Program

Pyx Health, which provides a social health platform to address loneliness, launched a new program to improve consumer self management of health conditions. The customizable program supports a wide range of conditions, including diabetes, hypertension, pregnancy, and much more.

Pyx Health provides trained mentors and health coaches who offer companionship, techniques to reduce anxiety and stress, and help in navigating the treatment process. Pyx Health also identifies social determinants of health (SDOH) needs that may impact health outcomes – such as inadequate housing, food, or health care access – and connects members with appropriate resources.
