Community Care Cooperative Expands Its Medicare Shared Savings Program With Four More FQHC Partners
Community Care Cooperative (C3), an accountable care organization (ACO) founded and governed by Federally Qualified Health Centers (FQHCs), entered new partnerships with two FQHCs in California, one in the District of Columbia, and one in North Carolina. C3 participates in the Centers for Medicare & Medicaid Services' Medicare Shared Savings Program (MSSP), which the four FQHCs will join. C3 shares in the risk and reward across 43 FQHCs’ value-based agreements, representing more than 45,000 Medicare beneficiaries.
The four new participating FQHCs are:
Bread for the City in the District of Columbia
Shasta Cascade Health Centers in California
