Koda Health & Medical Home Network Partner To Expand Access To Advance Care Planning Tools For Federally Qualified Health Centers
Koda Health, an artificial intelligence (AI)-enhanced digital advance care planning (ACP) platform, announced a new partnership with Medical Home Network (MHN) to expand access to care planning (ACP) to federally qualified health centers (FQHCs). MHN will offer Koda Health's scalable, consumer-centered ACP solutions to select FQHC partners in the MHN network. The rollout began in the second quarter of 2025 across FQHCs in Oklahoma with the intention to expand to additional sites. Financial terms of the partnership were not disclosed.
