Founded in 2011, Coordinated Behavioral Care (CBC) is a non-profit, provider-owned but member-led clinically integrated network of 77 behavioral health and human services organizations across New York State. CBC delivers behavioral health care, primary care, care coordination, housing supports, and social care through its Independent Practice Association (IPA) and Health Home infrastructure. CBC’s mission is to “[leverage] community partnerships to coordinate integrated medical and behavioral health interventions that, coupled with a specialized emphasis on social determinants of health, [to] promote a healthier New York.”
