Hillside Children’s Center Launches The Next Steps Critical Time Transition Program

Hillside Children’s Center announced the opening of the Next Steps Critical Time Transition Program (CTTP), a collaboration with the University of Rochester Medical Center, Rochester Regional Health, and other regional hospitals in New York.

Hillside is the first agency in New York to admit youth into the program, which provides enhanced care-management services for youth with complex needs, as well as transitional residential beds, as needed, as the youth journey from hospital stays back to their homes and families.
