Rural Health Transformation Program State Project Abstracts

In December 2025, Rural Health Transformation (RHT) published a document of applications submitted on behalf of each state for the Program State Project Abstracts. States were required to submit a one-page summary of their proposed project including its purpose and anticipated outcomes for the purposes of public information sharing.
