Habitat Health With Kaiser Permanente Opens South Los Angeles PACE Center For Adults Age 55 & Over

Habitat Health, in collaboration with Kaiser Permanente, announced the opening of its first center offering the Program of All-Inclusive Care for the Elderly (PACE) in the Los Angeles area. The center serves eligible adults age 55 and older in South Los Angeles. The program offers primary and specialty care, transportation, in-home support, meals, prescriptions, and social activities to help older adults with disabilities remain living in their communities. These services are typically available at no cost to PACE participants who qualify for both Medicare and Medi-Cal (California’s Medicaid program).
