DaVita Announces New Value-Based Partnership With Humana To Expand Care For Individuals With Chronic Kidney Disease
DaVita announced a value-based care agreement to provide coordinated care to Humana Medicare Advantage members with chronic kidney disease (CKD) stages 3B through 5.
The partnership began July 1, 2026, and expands the organizations’ existing collaboration in end-stage kidney disease to include earlier stages of the condition.
Through DaVita Integrated Kidney Care (DaVita IKC), consumers receive coordinated care addressing cardiovascular, kidney, and metabolic health. The program is intended to reduce fragmented care and avoidable hospital admissions.
