C5.Consumer Assessment of Healthcare Providers and Systems – Health Plan Measures
In December 2013, the federal Agency for Healthcare Research and Quality (AHRQ) released nine core measures for assessing the quality of behavioral health care services integrated into primary care. The measures were developed and validated for use in various quality-of-care applications. Core measures align closely with the Integration Framework. The fifth measure is C5 is Consumer Assessment of Healthcare Providers and Systems - Health Plan Measures. C5's original title is: "Reporting Measures for the CAHPS® Health Plan Survey 4.0". It was designed to measure coordinated care in general and can be adapted to measure integrated care.
