Tailoring New York’s Health Home Model For Children
The New York State Medicaid program is preparing to implement health homes for children using currently existing health home provider organizations, but to tailor the model the state may consider provider organizations that specialize in services for children and youth. The children’s health homes would begin enrollment in January 2015.
