Together4Health: A Response To The State Of Illinois Innovations Project For Care Coordination Entities

In February 2013, Together4Health submitted a winning proposal to become a Care Coordination Entity (CCE). The lead partner is Heartland Health Organization, Inc.. The target population is adults and seniors with disabilities, including those with serious mental illness (SMI) and people who are dually eligible, in Cook County. The model will test care coordination based on the health home setting, including hard to reach populations such as the homeless.
