New York Medicaid Draft Health Home Application to Serve Children

On June 30, 2014, New York Medicaid released a draft application for children’s health homes, which clearly signals the state’s plans for the statewide rollout and the types of provider organizations eligible to become Health Homes for children. The New York Department of Health (DOH) seeks comments and letters of intent through July 30, 2014. The final application is slated for release on August 29, 2014, and final applications will be due by September 30, 2014.
