Indiana Family & Social Services Administration Division of Disability and Rehabilitative Services Draft Health Home Provider Qualifications &Standards

The Indiana Family and Social Services Administration (FSSA) Division of Disability and Rehabilitative Services (DDRS) is planning to implement health home services for individuals with developmental disabilities who are at risk for developing another chronic health condition. According to draft qualifications and standards presented to provider organizations in February 2013, DDRS is proposing a statewide implementation and anticipates that 50 of its 127 Medicaid residential habilitation services (RHS) provider organizations will become health homes. The provision of care coordination through health homes will be administered as a State Plan Medicaid service, not Medicaid waiver.
