The federal Centers for Medicare & Medicaid Services (CMS) responded to questions about how to use new Medicare transitional care management codes. The new codes became available on January 1, 2013. The new codes enable reimbursement for physicians providing post-discharge follow-up care to patients during the first 30 days after leaving a hospital or skilled nursing facility.
