Minnesota’s Statewide HCBS Transition Plan

On November 17, 2014, the Minnesota Department of Human Services (DHS) released the Medicaid statewide transition plan governing home- and community-based services (HCBS) to ensure by December 2018 that HCBS waiver services are provided in qualifying settings that comply with federal rules for community settings, person-centered planning, integration, and choice. The transition plan applies to Minnesota’s 1915(c) waivers: the Brain Injury waiver, the Community Alternative Care waiver, the Community Alternatives for Disabled Individuals waiver, the Developmental Disabilities waiver, and the Elderly waiver.
