Medicaid Redesign Team Supportive Housing Health Home Pilot Project: Request for Applications #1310310853

The New York State Department of Health (DOH) Medicaid Redesign Team (MRT) released this request for applications (RFA) on March 11, 2014, with applications due by May 23, 2014. The state sought applications from supportive housing provider organizations for a pilot project in which supportive housing providers will partner with of one or more of the 34 designated Medicaid Health Homes operating in 62 counties. In early December 2014, the DOH awarded $4 million in contracts to 10 supportive housing provider organizations.
