In July 2013, the Center for Integrated Health Solutions released a report discussing Medicaid health home models in five states currently including serious and persistent mental health condition or substance use disorder as eligible chronic conditions under their health home benefit: Missouri, New York, Ohio, Oregon, and Rhode Island. The report focus is largely on Missouri and Rhode Island, which were the first states in the nation, respectively, to receive federal approval for health home services coverage.
