August 1, 2006 The Community Transition Program is a program developed by the St. Joseph Probate Court; it is funded by a grant from the Indiana Department of Correction. The program is intended to provide a structured and supportive release from institutionalization into the community for youth involved with the juvenile justice system. The program provides parents with services to establish healthy family structures that provide support and appropriate supervision.
