Medical Home Scan

November 2008 Between January and October of 2008 National Academy of State Health Policy staff conducted a scan of state Medicaid and State Children's Health Insurance Plan (SCHIP) efforts to advance medical homes. A variety of efforts were identified. Some had implementation dates in the 1990s.
