January 2005 Medi-Cal Managed Care Expansion Aged, Blind, Disabled, & Long-Term Care Populations As the State of California planned to expand managed care into new populations, it developed two primary expansion models: Two-Plan and Geographic Managed Care (GMC)   County Organized Health Systems (COHS) The Two-Plan and GMC models use the same set of aid codes.
