November 2005 Arizona 12th Annual Child Fatality Review: November 2005 There were 1,048 child deaths reported in Arizona during 2004 and 1,031 (98 percent) of these deaths have been reviewed for his report. The Child Fatality Program created the checklist to assist local teams in determining whether or not a death was preventable and to identify the most common factors present in childhood deaths.
