January 2005 The Community Initiative on Depression: Report From a Multiphase Work Site Depression Intervention In 2000, a multidisciplinary health care coalition implemented a multiphase workplace depression initiative in Kansas City, Missouri. The coalition researchers administered a survey to determine employees' knowledge of depression, their attitudes towards depression and access to treatment. The survey, a 22-item, self-administered on-line or paper, was given to 38,945 employees of 13 large employers in the Kansas City, Missouri area. Surveys were completed by 6,399 people.
