Introducing Health Homes: Improving Care for Medicaid Recipients with Chronic Conditions
Overview of the Health Homes program in New York released by the New York Department of Health in 2012. The Health Homes will improve the health care provided to both fee-for-service (FFS) and Managed Care Plan (MCP) members of the Medicaid Program (who are medically needy) and will save money by reducing preventable hospitalizations, emergency room visits and unnecessary care.
