Horizon House Selects CoCENTRIX To Provide Mobile Care Coordination Solutions

Philadelphia-based Horizon House selected the CoCENTRIX Coordinated Care Platform (CoCENTRIXccp) as their integrated electronic health record, care coordination, and billing solution, connecting Horizon House's 200 programs and connecting providers to primary care and pharmacies. Horizon House will utilize the CoCENTRIXccp to meet Meaningful Use requirements. Horizon House provides behavioral health, homeless, and intellectual disability services to individuals in six counties throughout Pennsylvania and Delaware.
