Connected Care Data Template

January 1, 2009 In 2009, the University of Pittsburgh Medical Center (UPMC) published an integrated care ("connected care") data template for behavioral health clients, which was made available on the Centers for Medicare & Medicaid Services (CMS) Integrated Care Resource Center website. The template includes fields pertaining to primary and behavioral health, member contacts, service utilization records, and care coordination triggers, access to care needs, and associated interventions.
