Introduction To The Ohio Patient-Centered Medical Home Care Delivery & Payment Model

On January 14, 2016, the Ohio Office of Health Transformation (OHT) announced that the state will implement a planned patient-centered medical home (PCMH) model statewide in 2016, two years ahead of schedule. This document presents the details. In May 2015, OHT convened a PCMH Design team to develop a payment model for adoption across Medicaid and commercial health insurance.
