New Jersey Home Health Readmissions Report
The report was released in January 2015 by the Home Care and Hospice Association of New Jersey (HCHANJ). The research was conducted by Quality Innovation Network on behalf of HCHANJ, which commissioned the report to understand the relationship between home health services and the stateÂ’s higher than average 30-day readmission rates among fee-for-service Medicare beneficiaries.
