New York OPWDD FIDA-IDD Demonstration Requirements For Assessment, Service Planning & Authorization, & Ongoing Care Management (IDT Policy)

It presents demonstration requirements for assessment, service planning and authorization, and ongoing care management for the OPWDD Fully-Integrated Duals Advantage Plan -Intellectual and Developmental Disabilities (FIDA-IDD) plan operated by Partners Health Plan. The services went live on April 1, 2016.
