In this funding opportunity released in August 2015, the federal Centers for Medicare & Medicaid Services (CMS) sought expressions of interest from organizations participating in phase one of the Initiative To Reduce Avoidable Hospitalizations Among Nursing Facility Residents. In phase two, the organizations will implement new payment models intended to help reduce the prevalence of hospitalization among nursing facility residents.
